Orders MUST be received by 2:30 pm EST, Monday Friday, using the
ORDER REQUEST AND CONFIRMATION FORM.

- TO CANCEL AN ORDER:
You MUST cancel an order in writing using the
ORDER REQUEST AND CONFIRMATION
FORM, this form MUST be received
within forty-eight (48) hours prior to the expected delivery date.
Shipping and Delivery
|IS REQUIRED FOR THE ORDER PROCESSING for local deliveries within the U.S.
|IS REQUIRED FOR THE ORDER PROCESSING for deliveries within the U.S.,
PUERTO RICO and HAWAII
|IS REQUIRED FOR THE ORDER PROCESSING for deliveries to other WORLDWIDE destinations.
BioGenetics reserves the right to ship and deliver orders via Federal Express or thru a reliable network courier service.
Saturday shipment via Federal Express can be arranged, provided the courier service is available in your area. There is an additional charge for Saturday shipments.
BioGenetics does not ship orders on Fridays for a Monday delivery.
For an assisted reproductive procedure scheduled for a Monday, it is recommended, that, the order be shipped for delivery on the Friday before the procedure.
NO SUNDAY or HOLIDAY DELIVERY SERVICE IS AVAILABLE
Orders are shipped in "Biological Dry Shippers" (BDS) charged with liquid nitrogen vapor.
The BDS keeps the vials frozen for five (5) days, inclusive of the shipping day.
A BDS expiration date is printed at the top of the packing slip enclosed with every shipment.
The BDS MUST be returned to BioGenetics within ten (10) days from the shipping/delivery date to avoid a daily rental charge of $35.00 per day.
BILLING and CREDIT POLICY
- Terms: Full payment with order request and confirmation.
- Fees are subject to change without prior notice.
- Shipping/Delivery Fees: Fees will vary for deliveries outside the U.S.A.
BioGenetics cannot accept or issue a credit for the return of unused Donor vial(s).
BioGenetics will accept Donor vial(s) on behalf of a Client/Recipient and place them into storage for use by the Client/Recipient of origin.
Specific documents must be completed and submitted prior to the return and acceptance of Donor vial(s) for storage on behalf of Client/Recipient of origin.
Return shipment fee will apply. No Storage fee for first twelve (12) months.
Consideration for Credit may be issued for Donor vial(s) with indices that fall below the guarantees/standards set forth in the “Medical Provider Service Agreement For CryoBanked Donor Sperm" for the use of Cryporeserved Donor Sperm in Assisted Reproduction Technology.
Physician/Medical Facility MUST notify BioGenetics within five (5) business days of the receipt of the shipment and within twenty-four (24) hours of thawing the Donor vial(s) to qualify for credit eligibility.
This notification MUST be followed up using one of BioGenetics Product Complaint Report Form.
For Donor vial(s) that are held beyond the five (5) day period, please refer to Section C under “Warranties” in the Medical Provider Service Agreement For CryoBanked Donor Sperm, for specific directives regarding credit policies.
