WHY should I cryobank?

It is widely documented and recognized that some surgical procedures or adjunct treatments for cancer may have adverse effects on reproductive cell(s) & tissue such as the ovaries which holds a woman’s entire supply of immature follicules/oocytes (eggs).

Surgery, chemotherapy, radiation therapy and many treatments may diminish the quantity and quality
of reproductive cell(s) & tissue. The use of a reproductive cell and tissue bank is a personal decision that only YOU can make.

Today, fertility preservation is an option available to females facing surgery or adjunct treatments when diagnosed with any of the following diseases:

Solid Tumors Spinal cord injury
Bone-marrow, stem cell transplant  Immunosuppressive drugs
Non-Hodgkin’s Lymphoma    Hodgkin’s Lymphoma
Bladder Cancer Colon Cancer

Other reasons you may consider:

Long-term Family planning is always an option for individuals that are career minded.

Hazardous occupations which may indirectly affect ovarian function due to job related physical hazards that may directly impair the reproductive system.

Toxic environments which may indirectly affect ovarian function due to related exposure to well established chemicals that may affect or directly impair the reproductive system.

What is Cryopreservation?

Cryopreservation is the process of ultra low temperature freezing of living cell(s) and tissue.

At SEBNJ reproductive cell(s) & tissue are stored and maintained at ultra low temperature maintained
by a continuous supply of Liquid Nitrogen.
Most cell(s) and tissue, including ovarian tissue and oocytes are able to continue to function in the natural way as they were before the freezing process.
The cryopreservation of cells and tissue has NOT been optimized universally to guarantee functionality of cryopreserved cells and/or tissue. The survival rate of cells and tissue cannot be totally controlled.

What reproductive cell(s) & tissue can I cryostore?

Ovarian tissue and Oocytes (women eggs) can be cryostored.
To accomplish this procedure it will require you to be under the care of a Reproductive fertility specialist.
The Reproductive fertility specialist will provide you with the best fertility preservation options available to you.   

Currently, three options are available
1) Ovarian Tissue retrieval surgically followed by cryopreservation. 
2) Oocytes (woman's eggs) are retrieved surgically and cryopreserved.  
3) Oocytes are retrieved and under-go “In-vitro” fertilization to create an embryo(s) which are in turn cryopreserved.

These procedures are performed at the Reproductive Fertility Specialist facility.
Finally, the cryopreserved Ovarian Tissue and/or Oocytes (woman's eggs) sample(s) will be transported for cryostorage at our facility SEBNJ.

When should I have my retrieval procedure performed?

Timing is everything…

We recommend that you explore your options to preserve your fertility with your Physician.
It is preferable for the collection of reproductive cell(s) & tissue to be started prior to any treatment, surgery or immune-suppressive drugs.

Our Client Depositor program provides individuals the ability to cryopreserve (freeze) and store their reproductive cells & tissue, such as ovarian tissue and/or oocytes for future use.

Reproductive cell & tissue banking allows women to choose the most appropriate time of their life to have children. Current advances in the field of Assisted Reproduction have allowed Clients to achieve a number of pregnancies using their own cryopreserved reproductive cells & tissue.

Why are the laboratory tests mandatory at SEBNJ?

Laboratory testing is mandatory of each and every Client Depositor.

Client Depositors vials or straws are segregated based on the out-come of the laboratory test results to protect and minimize the transmission of an infectious disease to the eventual Recipient.

For example: Client Depositors who test positive for Hepatitis are cryostored separately from
Client Depositors who test negative for Hepatitis.

Mandatory Client Depositor laboratory testing includes the following:

HIV 1 & 2 Antibodies Hepatitis B Core Antibody
HTLV I & II  Hepatitis C Virus Antibody
Blood Group/Rh Hepatitis B Surface Antigen
HIV-1/Hepatitis B/ Hepatitis C (NAT) Syphilis Serology (RPR) Screen

 

What are the fees for services?

  • Laboratory testing fees will be billed by the refrence laboratory to the insurance carrier
    or directly to the Client Depositor
  • Annual Storage fee
  • Shipping/Delivery Charges, when applicable
  • Administrative fees, when applicable

What documents are provided to the Client Depositor?

The Client Depositor will be the sole individual/person entitled to initiate the process of cryopreservation and the storage of her Reproductive Cell(s) & Tissue.

SEBNJ requires that the Client Depositor (Bailor) to complete the following documents:

  1. Referral/Request form for Reproductive Cell(s) & Tissue Transfer
  2. Request, Consent, Waiver for "Biological Shipper" Rental/Transport/Transfer of
    Reproductive Cell(s) & Tissue
  3. Request for Storage and Attestation for the Origin of Reproductive Cell(s) and/or Tissue
  4. Final Agreement for Reproductive Cell(s) & Tissue Storage
  5. Absentee Affidavit (when necessary)
  6. Test Request for Mandatory Testing for Reproductive Cell(s) & Tissue Storage (when necessary)
  7. Credit Card Authorization
  8. Notice for Withdrawal of Reproductive Cell(s) & Tissue from Storage (as/when needed)
  9. Notice for Destruction of Reproductive Cell(s) & Tissue (as/when needed)

Insurance coverage

Your right to obtain the best and most appropriate form of medical care should not be limited by your health insurance coverage.

SEBNJ has been dedicated for the last fifteen (15) years to provide the highest level of care possible to provide Reproductive Cell & Tissue banking services to all Clients requiring to cryostore reproductive cells and tissue.

The insurance industry has designated a CPT code (common provider term) for the cryostorage of reproductive cells & tissue. The extent of coverage and reimbursement varies from carrier to carrier and from plan to plan.  Clients should contact their plan Administrator or Insurance Provider to verify the extent of their insurance plan coverage. If you have any questions, or would like to discuss your personal healthcare coverage plan further, please do not hesitate to contact our office so that we may guide you.

We recommend that you have your specialized care physician provide you with either a letter and/or prescription recommending that you cryopreserve (freeze) and cryostore your reproductive cells & tissue for future use prior to chemotherapy, radiation, surgery, and/or immunosuppressive drugs.

SEBNJ supports the efforts of "Fertile Hope" and the Lance Amstrong "Live Strong" foundation and offers "Tissue Bank Cares™ (TBC) fees.
Our “Tissue Bank Cares (TBC)™” fees are extended to all financially challenged Client Depositors.     
These charitable “Fees for Services” are based on the financial information requested by SEBNJ to corrolate with the current US Federal charity guidelines.

These fees are reviewed and updated on a yearly basis.
"Tissue Bank Cares™ (TBC)" "is a program solely offered at SEBNJ and its affiliates.

How to schedule an appointment?

The procedure for the transfer/transport of the various reproductive cell(s) & tissue will be answered on an individual basis.

To allow for optimum quality assurance, reproductive cell & tissue specimen(s) must be transferred/transported to the cell and tissue bank following strict procedures and guidelines.
